It is customary to pay for services as treatment is rendered unless specific payment arrangements have been made with the front office prior to your appointment.

We are assigned providers for United Concordia and Delta Dental Insurance Companies and we electronically file dental insurance claims as a courtesy for all our patients, however, the patient's co-pay is to be paid at the time of service. For major dental treatment over $200.00, we will pre-authorize with your insurance company so that you will have an estimate of what they are willing to pay. You will receive a written estimate of follow-up treatment from our office of expected charges. If you have any changes with your insurance company please notify the office at your earliest convenience to avoid delayed payment to your account.
